Service requisition — testing

This is our service requisition form Rev.02, online. Fill it in once and completely and we can quote without sending anything back and forth. Everything here is what we need before we can price the work.

Two fields people usually get wrong

The name and address for the test report is often not the same as the contact person — it is the entity the report is issued to, and it must be in English. And the mounting condition changes an absorption result substantially, so state it in the material row rather than leaving us to assume.

5. Material details

ISO 354 and ASTM C423 need a continuous 10–12 m² specimen, and the mounting condition changes the result — state it here.

6. Handling

Drawings or datasheets: note them here and email them to info@altbkk.com quoting your company name.

Goes to info@altbkk.com. Your details are used to quote and carry out the work, nothing else. Your draft is saved in this browser until you submit.
